Please upload the following forms and documents.
- Parent Questionnaire
- Student Questionnaire
- Information Release Parent Consent Form
- Most recent 2021 primary school report (Year 4 Semester 1)
- Verification Documentation
- Any medical reports or learning support information, including:
- Paediatrician or allied health reports
- Primary school learning plans
- General information relating to how we can best support your child’s learning, social and emotional needs at school

Please note if all forms and required documentation are not received by 4pm Friday 30 July, your interview may be cancelled.
We are a Circular Economy school and would prefer to receive your paperwork electronically. However if you choose to print your forms you can either:
- deliver to Enrolments Office, Hillbrook, 45 Hurdcotte Street, Enoggera between 7:30am and 5:00pm, or
- mail to Enrolments Office, Hillbrook, Reply Paid 469, Everton Park Qld 4053
